PacificSource will follow CMS guidelines for billing 99497 and 99498.  Please add modifier 33 to these codes when billing along with an AWV to show Preventive, this will apply no member cost share.  If these codes are billed with an AWV as stand alone codes, members may be responsible for copays and coinsurance.  Please see the link below for the CMS guidelines. Please contact your Provider Service Representative should you have any additional questions.
