2018 Medication Therapy Management Program (MTM)

This is a free program for eligible members


Description of Services

PacificSource Medicare offers Medication Therapy Management services to eligible Medicare Part D members. Through our MTM program, we have identified specially trained Pharmacists throughout our service area. Our pharmacists do much more than dispense drugs. They are there to help you get the best results from your medications while keeping out-of-pocket costs down. By using an MTM Pharmacist, you can get the extra attention you need to keep your medications on the right track.


Eligible MTM Member

The PacificSource Medicare MTM program uses an opt-out enrollment method. If you are eligible for MTM services, you will automatically be enrolled and receive an introductory letter from PacificSource Medicare. We will enroll you in the MTM program if you meet all the following criteria:

  • Members must have at least three chronic diseases

    Examples include:

    Arthritis
    Osteoporosis
    Chronic Heart Failure (CHF)
    Diabetes
    Dyslipidemia
    End-Stage Renal Disease (ESRD)
    Hypertension
    Bipolar Disorder
    Chronic / Disabling Mental Health Conditions
    Depression
    Schizophrenia
    Asthma
    Chronic Lung Disorders
    Chronic Obstructive Pulmonary Disease (COPD)
    Anticoagulation
    Chronic Alcohol and Other Drug Dependence
    Hepatitis C
    HIV/AIDS
    Multiple Sclerosis
    Neurologic Disorders
    Parkinson's Disease
    Severe Hematologic Disorders

  • Members must currently be taking a minimum of three Part D drugs
  • Members must be expected to spend at least $3,967.00 per year on medications

Services offered in an MTM


Eligible PacificSource Medicare members will receive the following important services at no additional cost:

  • Comprehensive Medication Review (CMR)
    Your MTM Pharmacist will meet with you to review all your medications. This will help identify any duplications or conflicts and help to organize your medication schedule.
  • Prescriber Consult
    Your MTM Pharmacist will talk with you and your doctor(s) to resolve any problems found with your medications.
  • Non-prescription Consult
    Non-prescription drugs (over-the-counter) can resolve minor ailments easily and inexpensively. Your MTM Pharmacist will help you to treat a variety of conditions.
  • Drug Information
    Your MTM Pharmacist will talk to you about the purpose of your medications and correct uses. Your MTM Pharmacist will also follow-up with you or your provider as needed to resolve drug related problems such as drug interactions, drug side effects and drug dosing.

Written Medication Summary
After your annual CMR, you will receive a written summary of the medication information either at the end of the review of your medication or in the mail.

Quarterly Targeted Medication Reviews (TMRs)
Every quarter, if your MTM pharmacist notices any new medication issues, they may contact you or your provider to ensure safe and cost-effective medication use.

  • October 1 - February 14: 8:00 a.m. to 8:00 p.m. local time zone, seven days a week.
  • February 15 - September 30: 8:00 a.m. to 8:00 p.m. local time zone, Monday - Friday. During this time of the year, please leave a message on weekends, holidays, and after hours. We will return your call the next business day.

Where can I find an MTM Pharmacist?

Are all pharmacists in the PacificSource Medicare network also MTM Pharmacists?

  • Not all pharmacists in the PacificSource Medicare network are MTM Pharmacists. PacificSource contracts with pharmacists who are uniquely positioned to work with you and your doctor to improve your medication regimens.

What if I do not want to be enrolled in the MTM program?

  • You can always disenroll if you no longer want to receive MTM services. Please call our Customer Service department and tell us you no longer want to be enrolled in the MTM program.

Can I use my regular pharmacy and still visit an MTM Pharmacist too?

  • The MTM is a free service offered by participating pharmacists to qualifying members. This service is in addition to your plan benefits. You may continue to use any PacificSource Medicare participating pharmacy for your prescriptions.

Will I pay additional costs for using an MTM Pharmacist?

  • PacificSource Medicare offers these services at no additional cost to you.

Can an MTM Pharmacist save me money?

  • Similar medications can have large cost differences. Your MTM Pharmacist can help you take the least costly, most effective medications.

Will my doctor know if I need to make any changes to my prescriptions?

  • Your MTM Pharmacist may make recommendations to you and your doctor(s) but only your doctor can change your prescription.

Will I be required to use an MTM Pharmacist?

  • Your MTM Pharmacist is an added service of membership. They are there to help, but you are not required to use them.

In general, beneficiaries must use network pharmacies to access their prescription drug benefit, except under non-routine circumstances. Quantity limitations and restrictions may apply. The Formulary and pharmacy network may change at any time. You will receive notice when necessary.
