PacificSource Medicare offers Medication Therapy Management services to eligible Medicare Part D members. Through our MTM program, we have identified specially trained Pharmacists throughout our service area to work with you and your doctor to help you get the best results from your medications while keeping out-of-pocket costs down. By using an MTM Pharmacist, you can get the extra attention you need to keep your medications on the right track.
The PacificSource Medicare MTM program uses an opt-out enrollment method. If you are eligible for MTM services, you will automatically be enrolled and receive an introductory letter from PacificSource Medicare. We will enroll you in the MTM program if you meet all the following criteria:

Written Medication Summary
You will receive a written summary of the medication information you talked about with the pharmacist either at the end of your in-person review or in the mail.
Quarterly Targeted Medication Reviews (TMRs)
Every quarter, if your MTM pharmacist notices any new drug therapy problems or improvements that can be made to the medicines you take, they may contact you or your provider to talk about it.
Getting started is easy and is at no cost to you
MTM program enrollment is automatic. Simply watch for the program welcome letter sent to you or accept the telephonic offer to schedule an in person or telephonic review.
