2023 Medication Therapy Management (MTM) Program

Description of services

PacificSource Medicare offers Medication Therapy Management services to eligible Medicare Part D members. Specially trained pharmacists will work with you and your doctor to help you get the best results from your medications while keeping your costs down. By using an MTM pharmacist, you can get the extra attention you need to keep your medications on the right track to keep you doing the things you love.

About eligibility

If you are eligible for MTM services, you will automatically be enrolled and receive a Welcome Letter from PacificSource Medicare. We will enroll you in the MTM program if you meet all the following criteria:

  • You have at least three chronic diseases. Examples include:
    • Anticoagulation
    • Bone Disease-Arthritis-Osteoporosis
    • Chronic Heart Failure (CHF)
    • Diabetes
    • Dyslipidemia
    • Hypertension
    • Neurologic Disorders
    • Respiratory Disease-Chronic Lung Disorders
  • You are currently taking at least five Part D chronic or maintenance drugs.
  • You expect to spend at least $4,935 per year on drugs.
  • PacificSource Medicare members who are taking specific types of controlled medications may be identified as At-Risk Beneficiaries (ARBs) under the Drug Management Program (DMP) and enrolled in the MTM program without meeting other criteria.

Services offered in the MTM program

We know everyone's life gets busy but we encourage you to make time for yourself so you can feel good and keep doing those things you love. Even if you aren't taking any new medications, an annual medication review can help make sure you feel as good as possible. The services offered through the MTM program are designed to be as easy as possible – meeting you where you are while also giving you the peace of mind you deserve.

Comprehensive medication review to identify potential problems for members

PacificSource Medicare works with specially trained MTM pharmacists and their friendly customer service teams to schedule telehealth, phone, or in some cases face to face visits! During the medication review the pharmacist will review all the medications you are taking, as well as over-the-counter vitamins and supplements. This comprehensive medication review will help identify any duplications or conflicts, help to organize a complex medication schedule, or to just check in and provide a listening ear to answer questions.

Personal consult to find the best solutions for you

Your personal MTM pharmacist will talk with you and your doctor(s) or connect with your regular pharmacist to resolve any issues or questions that come up during your medication review. Your MTM pharmacist can also help coordinate medications prescribed by multiple doctors and to help with medication synchronization to minimize the amount of time you need to spend coordinating your medications.

Clear explanations about your drugs

Your MTM pharmacist will

  • Talk to you about the best way to take your medications that helps you feel your best.
  • Explain why you need them or alternatives that may suit you better.
  • Answer any questions you may have about your medicine.
  • Follow-up with you or your provider as needed to resolve drug related problems, such as drug interactions, side effects, and doses.

Written medication summary

After your medication review you will receive a written summary of the drug information you talked about with the pharmacist at the end of your in-person review or in the mail after your review. The summary provided to you will list any follow up's as a reminder to what was discussed and can be used to share information with your doctor or specialists you may be seeing.

Quarterly targeted medication reviews help to keep your medications on track

Each quarter you are enrolled in the MTM program, your MTM pharmacist will review your drugs to see if there are any areas for improvement or drug therapy problems they can work with your doctor to solve. They may contact you or your provider to talk about your medications. The good news is, we have your back and you don't need to do anything to receive this free service.

Eligible members are automatically enrolled

To make it even easier, MTM program enrollment is automatic. Simply watch your mail for the program Welcome Letter sent to you with ways to contact your personal MTM pharmacist to complete your annual medication review. PacificSource Medicare also works with specially trained groups of MTM pharmacists whose friendly customer service teams will reach out to schedule a virtual or telephonic visit.

Where can I find an MTM pharmacist?

  • Watch your mail for your program enrollment Welcome Letter with your MTM pharmacist's contact information.
  • If you receive a call from us or one of our partners, accept the offer to schedule a complete medication review.
  • Contact your doctor's office or provider clinic. In some cases, your MTM pharmacist will work at the same location as your doctor.
  • Call our Customer Service team at the numbers listed above. They can help connect you with your personal MTM pharmacist.

Are all pharmacists in the PacificSource Medicare network also MTM pharmacists?

  • No. Not all pharmacists in the PacificSource Medicare network are MTM pharmacists. PacificSource contracts with specially trained pharmacists who will work with you and your doctor to improve your medication plan.

What if I do not want to be enrolled in the MTM program?

  • You can always opt out of the MTM program. Please contact our Customer Service team and tell us you no longer want to be in the MTM program.
  • Members who take or have used controlled medications may be automatically enrolled in Medication Therapy Management without meeting other program eligibility and opt-out limitations may apply.

Where can I dispose of unused prescriptions?

  • The U.S. Drug Enforcement Administration (DEA) and its partners collect and safely dispose of medications as a service to communities. We suggest visiting the DEA website for locations in the community that might be convenient for you:
  • Check with these locations in your community to see if they have drug disposal boxes:
    • Police stations
    • Pharmacies
    • Fire stations
    • Municipal buildings

Can I dispose of prescriptions at home?

  • Yes. When a take back option is not readily available, two options of disposing medications at home are:
    • Flush the Medication down a sink or toilet if the drug is on the FDA Flush List. Available online at: FDA.gov/drugs/disposal-unused-medicines-what-you-should-know/drug-disposal-fdas-flush-list-certain-medicines.
    • Trash Disposal. If the drug is not on the FDA Flush List, follow these steps:
      • Remove the drugs from their original containers and mix them with something undesirable, such as used coffee grounds, dirt, or cat litter. This makes the medicine less appealing to children and pets, and unrecognizable to someone who might intentionally go through the trash looking for drugs.
      • Put the mixture in something you can close (a resealable zipper storage bag, empty can, or other container) to prevent the drug from leaking or spilling out.
      • Throw the container in the garbage.
      • Scratch out all your personal information on the empty medicine packaging to protect your identity and privacy. Throw the packaging away.

Can I use my regular pharmacy and still visit an MTM pharmacist, too?

  • Yes. The MTM is a free service. This service is in addition to your plan benefits. You may continue to use any PacificSource Medicare in-network pharmacy for your prescriptions.

Will I pay additional costs for using an MTM pharmacist?

  • No. PacificSource Medicare offers these services at no additional cost to you.

Can an MTM pharmacist save me money?

  • Yes. Similar medications can have large cost differences. Your MTM pharmacist can help you take the least costly, most effective medications, to keep you feeling your best!

Will my doctor know if I need to make any changes to my prescriptions?

  • Yes. Your MTM pharmacist may make recommendations to you and your doctor, but only your doctor can change your prescription.

Will I be required to use an MTM pharmacist?

  • No. Your MTM pharmacist is an added service of membership. They are there to help, but you are not required to participate in the program.
