Select Wheelchairs – K0002, K0003, K0004, K0006, K0007

Wound Vacs – E2402, 97605

BiPaps – E0470, E0471

The following services/codes will be added to the Prior Authorization Grid:

Watchman Device – 0281T

Continuous Glucose Monitors – K0553, K0554
