For Medicare Advantage members who are being recommended for treatment with EpiFix skin substitute, PacificSource Medicare Advantage plans will begin requiring a prior authorization for this treatment. The associated code(s) will be added to our prior authorization grid (https://administration/AuthorizationGrid/Search) on July 11, 2022. Any questions can be directed to our Medicare Health Services department.
