Urgent Notice for Uprise (formerly known as RBH) Contracted Providers

Greetings!

Thank you for your continued care of our members. We are contacting our providers regarding a recent communication received from Uprise Health, formerly known as Reliant Behavioral Health (RBH). An independent provider association we have historically utilized for contracting some of our Behavioral Health and Alternative care providers. Uprise has decided to terminate their contract with PacificSource. Please disregard this email if you already have a direct contract with PacificSource. You may also disregard this email if you are not a Behavioral health or Alternative care provider contracted with Uprise Health.

What are your next steps?

Behavioral health:

If you would like to continue participating with PacificSource after 12/31/2022 and have not already signed a direct contract with us, please outreach your Provider Service team right away to request the forms needed to ensure you remain participating with PacificSource. Please note, these forms must be received within 30 days to ensure no disruption in contract status.  If we do not receive a signed contract back before the end of the year 2022, please know your participation status will terminate as per the Uprise decision.

Please complete and submit the following documents for the practice only:

  • Contract Interest/Information Form
  • Provider Roster
    • Use if there is more than one individual NPI billing under the Tax ID, or if you are a solo practitioner billing with a Type-II NPI
  • W9

Alternative care providers:

All Alternative Care providers will need to be credentialed and contracted by The CHP Group for participation in Commercial and Medicare. The CHP Group is a preferred provider organization and follows the same federal, state, and NCQA timelines as PacificSource for credentialing and contracting. Medicaid contracting will be directly with PacificSource, not via CHP and will require you to reach out to PacificSource directly for Medicaid.

Please contact CHP by emailing them directly. If you already participate with CHP, you will still want to verify your status with CHP and the next steps for opting to participate with PacificSource.
